Change/adding/dismissal of supervisor
In justified cases, a doctoral student may apply for:
- appointment, change or dismissal of supervisor,
- appointment, change or dismissal of assistant supervisor.
An appropriate application must be submitted to the Doctoral School along with the statement of the person(s) nominated as the candidate for the new supervisor.

In justified cases, doctoral student may apply for a change of supervisor or/and assistant supervisor. Once a change of supervisor(s) has been agreed, the appropriate application should be prepared according to one of the following templates:
The relevant application is to be submitted by the doctoral student to the Doctoral School together with the statements of the supervisor candidate. Document templates for supervisors:
- Statement of the academic teacher nominated as a supverisor.
- Statement of the academic teacher nominated as an assistant supverisor.

In justified cases, doctoral student may apply for the appointment of:
- second supervisor (if the second supervisor was not previously appointed) or
- assistant supervisor (if the assistant supervisor was not previously appointed).
Note that if an additional supervisor or assistant supervisor was selected, the appropriate application should be prepared according to one of the following templates:
- Application for appointment of second supervisor.
- Application for appointment of assistant supervisor.
The relevant application is to be submitted by the doctoral student to the Doctoral School together with the statements of the supervisor candidate. Document templates for supervisors:

- supverisor (if two supervisors were previously appointed) or
- assistant supervisor.
